Admission Documents

 

The following is a checklist of documents to be submitted during the application process:

•  Transcript or Report Cards for Current and previous year

•  Annual Assessment Test Scores (eg. SAT, IOWA State Assessment, etc.)

•  Birth Certificate

•  Social Security

Students applying for grades K-12:

•  Birth Certificate

•  Social Security

• Report Card

• IEP  (applicable)

• Annual Assessment Test Scores (eg. SAT, IOWA State Assessment, etc.)

• Health Insurance Card

Immunization Certificate (form 680) and a recent physical (form 3040) are required for all students and are due upon acceptance.
